Nurse Practitioner (NP)

HarmonyCares provides in-home primary care services to patients with complex healthcare needs through a team-based, physician-driven model. This Nurse Practitioner position is based in Williamsburg, Virginia, and involves delivering care Monday through Friday to high-acuity patients in their homes without hospital rounds or holiday work. The role operates within an Accountable Care Organization serving multiple states.

Responsibilities

• Provide primary care visits to high-acuity patients in their homes on a Monday through Friday schedule
• Focus on addressing individual patient needs with adequate time per visit rather than volume targets
• Work within a home-based primary care practice integrated with an overall physician-driven care model

Requirements

• Active Nurse Practitioner license in the state of Virginia
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Geriatric training or experience preferred
• Valid driver’s license with a good driving record
