Nurse Practitioner Trainer (IMPACT) positions are available with HarmonyCares, a national value-based provider of in-home primary care services for patients with complex healthcare needs. The role is based in Brentwood, Tennessee, and requires frequent travel to support the launch of new markets and the stabilization of existing ones across multiple states. The position focuses on training local teams, establishing clinical workflows, and delivering direct patient care in home settings under an integrated, team-based model.
Responsibilities
Travel to designated regions to assist with initial market setup, team launches, and operational rollout of new care locations.
Collaborate with IMPACT team members and local leadership to develop workflows, clinical protocols, and care delivery infrastructure aligned with company standards.
Provide on-site training and mentorship to local clinicians on home-based patient care practices and company care model principles.
Build relationships with local healthcare providers and community resources to support market establishment and ongoing operations.
Evaluate existing market performance and implement changes to address care gaps, standardize processes, and improve team outcomes.
Conduct comprehensive in-home health assessments, physical exams, diagnostic testing, and therapeutic interventions for patients with complex needs.
Create and adjust individualized care plans in coordination with the care team based on patient assessments and clinical findings.
Administer medications, injections, wound care, and other treatments while following safety and infection control protocols.
Educate patients and families on disease management, treatment options, and lifestyle modifications to support health outcomes.
Requirements
Current Nurse Practitioner license with ability to obtain and maintain licensure in all service states within six months of hire.
Active DEA license or eligibility to obtain one.
Active CPR or BLS certification.
Valid driver’s license with a good driving record.
Proficiency with electronic health record systems.
Geriatric training or experience.
Ability to adapt to varying weather conditions and residential environments.
Full range of body motion including patient handling and lifting, manual dexterity, eye-hand coordination, normal vision and hearing, and capacity for standing, bending, walking, and stair climbing.
Ability to lift and carry items weighing up to 50 pounds regularly.
Capacity to travel by automobile up to 150 miles daily in urban or rural areas and to travel for extended periods as needed.
