Patient Engagement Specialist

HarmonyCares is a national value-based provider of in-home primary care services for patients with complex healthcare needs, operating home-based practices across 14 states. The Patient Engagement Specialist role focuses on new patient acquisition through outbound outreach and scheduling of initial in-home appointments. The position is based in the United States.

Responsibilities

• Conduct outbound calls to eligible patients to facilitate scheduling of initial in-home appointments
• Provide detailed explanations of program services to patients
• Document call notes, scheduling details, patient feedback, and special needs accurately in electronic systems
• Expedite emergency clinical calls as needed
• Process new appointments or changes to existing appointments
• Meet department metrics including quality scores, acceptance rates, and number of patients engaged
• Participate in call quality monitoring processes and complete required ongoing training by deadlines
• Collaborate with corporate and market-based team members on patient service needs
• Perform other duties as assigned

Requirements

• High school diploma required
• Bachelor's degree preferred
• One or more years of experience exceeding individual and team goals in inside sales, telephonic sales, fundraising, or similar positions preferred
• Working knowledge of the healthcare industry preferred
• Ability to speak multiple languages fluently preferred
• Proven data-entry skills and ability to navigate multiple computer screens
• Ability to multi-task while handling competing priorities

• Compensation range of $20.10 to $25.69 per hour, based on skill set, experience, and qualifications
