Nurse Practitioner part-time Queens NY

This part-time Nurse Practitioner role is with HarmonyCares, a national value-based provider of in-home primary care services for patients with complex healthcare needs. The position is based in Queens, New York, and focuses on delivering primary care to high-acuity patients in their homes under a physician-driven model. Providers work Monday through Friday with no hospital rounds or holidays.

Responsibilities

• Provide primary care to high-acuity patients in their homes Monday through Friday
• Practice medicine with a focus on addressing individual patient needs rather than volume

Requirements

• Licensed as a Nurse Practitioner in New York
• Federal DEA License or eligibility
• Board Certification
• Active CPR/BLS Certification
• Valid driver’s license and good driving record
• Geriatric training or experience preferred
