Nurse Practitioner Primary Care Westchester NY

HarmonyCares is hiring a Nurse Practitioner for primary care in Whitestone, New York. The role delivers in-home primary care services to high-acuity patients with complex healthcare needs as part of a national value-based provider operating home-based practices in 14 states under a team-based, physician-driven model. The schedule runs Monday through Friday with minimal call responsibilities, no hospital rounds, and no holidays.

Responsibilities

• Provide primary care to high-acuity patients in their homes
• Allocate sufficient time per visit to address individual patient needs

Requirements

• Licensed as a Nurse Practitioner in New York
• Federal DEA License or eligibility
• Board Certification
• Active CPR/BLS Certification
• Valid driver’s license and good driving record
• Geriatric training or experience preferred
