Job Description

Nurse Practitioner PRN positions with HarmonyCares are available across Ohio, with the Youngstown area included. HarmonyCares operates as an Accountable Care Organization delivering in-home primary care services for patients with complex needs under a physician-driven model. The role focuses on annual health risk assessments conducted in residential settings or via telehealth.

Responsibilities

  • Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions along with physical, mental, and social needs
  • Obtain patient history, perform examinations, and determine diagnoses
  • Provide written documentation of patient visits according to NCQA standards
  • Measure and record patient vital signs when required
  • Place case management referrals and communicate with primary care physicians as needed
  • Communicate with patients, caregivers, agency nurses, other providers, and vendors to support accurate diagnosis
  • Perform all clinical duties while following OSHA Universal Precautions
  • Maintain patient confidentiality at all times
  • Attend required meetings, in-services, and committee participation as requested
  • Engage in professional development activities while maintaining licenses and affiliations

Requirements

  • Active and unrestricted nurse practitioner license to practice in coverage states
  • Board certification through American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA)
  • Active BLS Certification
  • Current enrollment in Medicare and Medicaid
  • Valid driver’s license with good driving record
  • Proficiency with electronic health record systems
  • Geriatric training or experience
  • Ability to adapt to varying weather conditions and diverse home or residential environments
  • Full range of body motion including patient handling and lifting, manual and finger dexterity, eye-hand coordination, normal visual acuity and hearing, plus standing, bending, walking, and stair climbing
  • Regular lifting and carrying of items weighing up to 50 pounds
  • Ability to ride in an automobile or van for up to 150 miles daily in urban or rural settings, with driving capability as needed
