Job Description

The Physician Leader (MD/DO) Primary Care role is with HarmonyCares Primary Care Medical Group in Houston, Texas. This position leads a clinical pod team focused on value-based care and quality outcomes for high-risk Medicare patients with chronic conditions through in-home visits Monday through Friday. The leader partners with a Market Manager in a dyad structure and serves as collaborating physician for advanced practice providers within a network of more than 250 providers serving over 85,000 patients across 12 states.

Responsibilities

  • Serve as on-site supervising physician for assigned advanced practice providers and physicians within the pod while maintaining collaborative practice agreements within state limits.
  • Conduct at least one ride-along observation with each collaborative advanced practice provider per quarter.
  • Provide complex patient visits as needed for supervised advanced practice providers.
  • Collaborate with community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators on pod activities.
  • Develop and deliver clinical education for advanced practice providers on evidence-based care and related topics.
  • Lead high-risk huddles and ensure documented care plans include input from all care management team members.
  • Perform 10 chart audits per month or the number required by state regulations, whichever is higher.
  • Review quality metrics and pod-level performance data to identify and implement improvements in HEDIS measures.
  • Review clinician capacity and productivity metrics and partner with site medical directors on corrective actions when needed.
  • Work with the regional medical director to identify care improvement opportunities.
  • Support resolution of service inquiries and patient complaints for the pod.
  • Assist pod providers with EMR efficiency.
  • Manage a panel of assigned patients and provide care for additional high-risk or complex cases as required, including cognitive assessments and physician face-to-face visits.
  • Deliver clinical education on accurate diagnosis, documentation, and chronic condition management.
  • Handle compliance matters and human resources resolutions as needed.
  • Conduct performance reviews and disciplinary actions for direct reports.
  • Complete additional duties assigned by the regional medical director related to pod performance and outcomes.

Requirements

  • Bachelor’s degree.
  • Doctor of Medicine or Doctor of Osteopathy degree.
  • Completion of an accredited residency program.
  • Unrestricted medical license.
  • Board certification required for all physicians hired or promoted on or after January 1, 2026, and must be maintained per organizational and regulatory standards.
  • Unrestricted DEA registration.
  • Active CPR certification.
  • Valid driver’s license with a good driving record.
  • Three years of experience as a physician collaborating with advanced practice providers (preferred).
  • Experience in geriatrics or with complex patients (preferred).
