Nurse Practitioner (NP) Primary Care

Nurse Practitioner (NP) Primary Care role with HarmonyCares, a national value-based provider of in-home primary care services for patients with complex healthcare needs. The position is based in Okemos, Michigan, and focuses on geriatric primary care for chronic conditions Monday through Friday with no weekends or evenings. HarmonyCares operates home-based practices in 14 states under an integrated, team-based, physician-driven model with the mission of delivering personalized, quality-based healthcare to patients who have difficulty accessing care.

Responsibilities

• Provide primary care to high-acuity geriatric patients in their homes Monday through Friday
• Manage chronic conditions with time allocated to address individual patient needs rather than volume targets
• Deliver care in home health, geriatrics, hospice, and primary care settings
• Maintain minimal call responsibilities with no hospital rounds or holidays

Requirements

• Licensed as a Nurse Practitioner in the state where the HarmonyCares office is located
• Federal DEA license or eligibility
• Board certification
• Active CPR/BLS certification
• Geriatric training or experience preferred
• Valid driver’s license and good driving record
