Physician Leader (MD/ DO) Primary Care

Job Description

HarmonyCares operates a value-based in-home primary care practice serving patients with complex healthcare needs in Flint, Michigan. The Physician Leader role directs a clinical pod of advanced practice providers focused on chronic condition management during weekday daytime hours with no weekends, evenings, or hospital rounds. The position maintains a dyad partnership with the Market Manager and functions as the collaborating physician for pod APPs.

Responsibilities

  • Provide on-site supervision for assigned APPs and physicians while maintaining collaborative practice agreements within state limits
  • Conduct quarterly ride-alongs with each collaborative APP and perform complex patient visits as required
  • Collaborate with community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators on pod activities
  • Lead high-risk patient huddles and ensure documented care plans involve all relevant team members
  • Complete at least 10 chart audits monthly or the number required by state regulation, whichever is greater
  • Review quality metrics and pod performance data to drive HEDIS improvements
  • Monitor clinician capacity and productivity; partner with site medical directors on corrective actions when needed
  • Sign DME orders, death certificates, and other documents on behalf of APPs per state regulations
  • Perform routine panel reviews with APPs and support EMR efficiency
  • Manage direct-report performance reviews, discipline actions, and compliance matters
  • Participate in patient care for the assigned panel including high-risk visits, physician face-to-face requirements, and cognitive assessments
  • Deliver clinical education on diagnosis, documentation, and chronic condition management

Requirements

  • Bachelor’s degree
  • Doctor of Medicine or Doctor of Osteopathy
  • Completion of an accredited residency program
  • Unrestricted medical license
  • Board certification required for hires or promotions on or after January 1, 2026
  • Unrestricted DEA registration
  • Active CPR certification
  • Valid driver’s license with acceptable driving record
  • Three years of experience as a physician collaborating with advanced practice providers (preferred)
  • Experience in geriatrics or management of complex patients (preferred)
