Physician Lead, Geriatric Primary Care in Fulton County, GA -$50k Retention Bonus

The Physician Lead, Geriatric Primary Care role at HarmonyCares delivers in-home primary care services for patients with complex healthcare needs in Atlanta, Georgia. HarmonyCares operates home-based primary care practices in 14 states under a team-based, physician-driven model. This position leads a clinical pod with a dyad partnership to the Market Manager, includes a $50k retention bonus, and requires no holidays, weekends, or hospital rounds.

Responsibilities

• Serve as on-site supervising physician for assigned advanced practice providers and physicians within the pod, ensuring collaborative practice agreements stay within state limits
• Complete at least one ride-along with each collaborative advanced practice provider per quarter and provide complex patient visits as needed
• Coordinate with community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators on pod activities
• Lead high-risk patient huddles and confirm documented care plans that include all relevant team members
• Perform 10 chart audits per month or the number required by state regulations, whichever is higher
• Review quality metrics and pod-level performance data to identify improvements aligned with HEDIS standards
• Monitor clinician capacity and productivity, then partner with site medical directors to implement countermeasures when targets are missed
• Sign durable medical equipment orders, death certificates, and other required documents on behalf of advanced practice providers per state regulations
• Conduct routine panel reviews with advanced practice providers and support EMR efficiency improvements
• Provide direct patient care for the assigned panel, including high-risk or complex cases and cognitive assessments
• Lead clinical education sessions on accurate diagnosis, documentation, and chronic condition management
• Handle discipline actions and performance reviews for direct reports
• Assist with service inquiries, patient complaint resolution, compliance matters, and HR issues as assigned by the regional medical director

Requirements

• Bachelor’s degree
• Doctor of Medicine or Doctor of Osteopathy
• Completion of an accredited residency program
• Unrestricted medical license
• Board certification required for hires or promotions on or after January 1, 2026, with ongoing maintenance per organizational and regulatory standards
• Active CPR certification
• Unrestricted DEA registration
• Valid driver’s license with acceptable driving record
• Three years of experience as a physician collaborating with advanced practice providers (preferred)
• Experience in geriatrics or with complex patients (preferred)
