Nurse Practitioner (NP) Primary Care

Nurse Practitioner role in primary care with HarmonyCares, a national provider of in-home primary care services for patients with complex healthcare needs. The position is based in Battle Creek, Michigan, and involves managing chronic conditions for an assigned patient panel on a Monday through Friday schedule with no weekends, evenings, or hospital rounds. Patient visits occur in the home setting using the Athena EMR and AI Scribe tools.

Responsibilities

• Manage chronic conditions for an assigned panel of high-acuity patients during in-home visits
• Complete charting during the workday with Athena EMR and AI Scribe support
• Focus on individualized patient care rather than visit volume targets

Requirements

• Active Nurse Practitioner license in Michigan
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Valid driver’s license with a good driving record
• Geriatric training or experience preferred
• Prior experience in home health, geriatrics, hospice, or primary care
