Nurse Practitioner (NP) Primary Care

HarmonyCares seeks a Nurse Practitioner for a geriatric primary care role providing in-home services to high-acuity patients with complex needs in Cedar Springs, Michigan. The position operates under a value-based, physician-driven model with an integrated team and covers a Monday-Friday schedule with no weekends, evenings, or hospital rounds. Providers manage their own patient panels focused on chronic conditions and complete charting during the workday using Athena EMR and AI Scribe.

Responsibilities

• Manage chronic conditions for an assigned panel of patients in home settings
• Deliver primary care visits to high-acuity geriatric patients Monday through Friday
• Document encounters in real time using Athena EMR and AI Scribe tools

Requirements

• Active Nurse Practitioner license in Michigan
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Geriatric training or experience preferred
• Valid driver’s license and good driving record
