Nurse Practitioner (NP)

HarmonyCares is hiring a Nurse Practitioner to deliver in-home primary care services to patients with complex healthcare needs in Reading, Pennsylvania. The organization operates value-based home-based practices in 14 states under an integrated, team-based, physician-driven model. This Monday-through-Friday role involves home visits with no hospital rounds or holidays and minimal call responsibilities.

Responsibilities

• Provide primary care to high-acuity patients in their homes
• Focus on direct patient care rather than volume-based metrics
• Allocate sufficient time to address individual patient needs during visits

Requirements

• Active Nurse Practitioner license in Pennsylvania
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Valid driver’s license with good driving record
• Geriatric training or experience preferred
• Direct experience in home health, geriatrics, hospice, or primary care
