Nurse Practitioner (NP) Primary Care

HarmonyCares seeks a Nurse Practitioner for a geriatric primary care role based in South Chicago, Illinois. The position involves delivering in-home primary care services to patients with complex healthcare needs under a team-based, physician-driven model. Providers see patients through 30-45 minute visits Monday through Friday with no weekends or evenings, using Athena EMR and an AI Scribe for charting.

Responsibilities

• Provide in-home primary care and chronic condition management to high-acuity patients
• Conduct 30-45 minute patient visits focused on HEDIS/STAR quality outcomes
• Document encounters using Athena EMR and AI Scribe during the workday

Requirements

• Active Nurse Practitioner license in Illinois
• Federal DEA license or eligibility
• Board certification as a Nurse Practitioner
• Active CPR/BLS certification
• Valid driver’s license and good driving record
• Geriatric training or experience preferred
