Nurse Practitioner (NP) Primary Care

HarmonyCares is hiring a Nurse Practitioner for a primary care role based in Harrisburg, Pennsylvania. The position centers on delivering in-home primary care to patients with complex healthcare needs under an integrated, team-based, physician-driven model. The schedule runs Monday through Friday with no hospital rounds and minimal call responsibilities.

Responsibilities

• Provide primary care to high-acuity patients in their homes
• Focus on patient-centered care with time allocated to address individual needs
• Work within home health, geriatrics, hospice, and primary care environments

Requirements

• Licensed as a Nurse Practitioner in Pennsylvania
• Federal DEA License or eligibility
• Board Certification
• Active CPR/BLS Certification
• Geriatric training or experience
• Valid driver’s license and good driving record
