PRN Family Nurse Practitioner ( Homevisits/Telehealth)

HarmonyCares seeks a PRN Family Nurse Practitioner to conduct home visits and telehealth encounters for patients with complex healthcare needs in Eugene, Oregon. The role operates within the company's home-based primary care model, which delivers value-based services across 14 states under a team-based, physician-driven approach. This PRN position focuses on annual risk assessments in residential settings and includes no hospital rounds or holidays.

Responsibilities

• Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions along with physical, mental, and social needs
• Obtain patient history, perform examinations, and determine diagnoses
• Provide written documentation of patient visits according to NCQA standards
• Measure and record patient vital signs as necessary
• Place case management referrals and communicate with primary care physicians as needed
• Coordinate with patients, caregivers, agency nurses, other providers, and vendors to support accurate diagnosis
• Perform all clinical duties while following OSHA Universal Precautions
• Maintain patient confidentiality at all times
• Attend required meetings, in-services, and committee participation as requested
• Engage in professional development activities and maintain professional licenses and affiliations

Requirements

• Active and unrestricted nurse practitioner license to practice in coverage states
• Board certification through ANCC, AANP, or NCCPA
• Active BLS Certification
• Current enrollment in Medicare and Medicaid
• Valid driver's license with a good driving record
• Proficiency with electronic health records
• Adaptability to varying weather conditions and residential environments
• Full range of body motion including handling and lifting patients, manual and finger dexterity, eye-hand coordination, normal visual acuity and hearing, plus ability to stand, bend, walk, and climb stairs
• Regular lifting and carrying of items weighing up to 50 pounds
• Ability to ride in an automobile or van up to 150 miles daily in urban or rural settings and to drive as necessary
• Geriatric training or experience
