Job Description

HarmonyCares is hiring a PRN/Per-Visit Family Nurse Practitioner to perform annual health risk assessments for patients with complex needs in Sussex County, New Jersey. The position is based in Newton and requires the practitioner to reside within Sussex County. Work occurs in residential settings or via telehealth under a physician-driven, team-based model with no hospital rounds or holidays.

Responsibilities

  • Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions plus physical, mental, and social needs
  • Obtain patient history, perform examinations, and determine diagnoses
  • Document patient visits according to NCQA standards
  • Measure and record vital signs when indicated
  • Place case management referrals and coordinate with the primary care physician as needed
  • Communicate with patients, caregivers, agency nurses, providers, and vendors to support diagnosis and care
  • Perform clinical duties while following OSHA Universal Precautions
  • Maintain patient confidentiality
  • Attend required meetings and in-services and participate in committees upon request
  • Complete professional development activities and maintain licenses and affiliations

Requirements

  • Active and unrestricted nurse practitioner license to practice in the coverage states
  • Board certification through ANCC, AANP, or NCCPA
  • Active BLS certification
  • Current enrollment in Medicare and Medicaid
  • Valid driver’s license with a satisfactory driving record
  • Proficiency with electronic health records
  • Ability to adapt to varying weather conditions and residential environments
  • Full range of body motion including patient handling and transfers, manual and finger dexterity, eye-hand coordination, normal visual acuity, and normal hearing
  • Capability to stand, bend, walk, and climb stairs
  • Regular lifting and carrying of items weighing up to 50 pounds
  • Ability to travel up to 150 miles daily by automobile in urban or rural settings
