Advance Practice Provider (NP /PA) - $140k/yr Salary + $7,500 sign on

HarmonyCares seeks an Advance Practice Provider (NP or PA) for its Bay City, Michigan location. The full-time role involves in-home primary care for patients with complex healthcare needs, with a schedule of Monday through Friday, 6-8 patients per day, and no weekends or evenings. HarmonyCares operates home-based primary care practices across 14 states under a value-based, team-supported model.

Responsibilities

• Provide primary care to high-acuity patients in their homes Monday through Friday
• Manage a daily caseload of 6-8 patients
• Deliver care focused on individual patient needs rather than volume targets
• Handle minimal call responsibilities with no holidays or hospital rounds

Requirements

• Licensed as a NP or PA in the State of Michigan
• Federal DEA License or eligibility
• Board Certification
• Active CPR/BLS Certification
• Directly related experience in home health, geriatrics, hospice, or primary care
• Geriatric training or experience preferred
