Primary Care Physician

HarmonyCares is hiring a Primary Care Physician to deliver in-home primary care services in Williamsburg, Virginia. The national value-based provider operates home-based practices across 14 states and focuses on patients with complex healthcare needs under an integrated, team-based model. The position centers on residential settings for elderly individuals and those with complex medical conditions, with a Monday-through-Friday schedule and no hospital rounds or weekend work.

Responsibilities

• Deliver primary health care and patient care management in a residential setting

Requirements

• Experience in geriatrics or complex patients preferred
• Active or unrestricted medical license and DEA registration
• Board certified or board eligible
• EMR experience
• Valid driver’s license and good driving record
