Family Nurse Practitioner PRN Telehealth in Ohio

Family Nurse Practitioner PRN Telehealth position with HarmonyCares in Youngstown, Ohio. HarmonyCares is a national value-based provider of in-home primary care services for patients with complex healthcare needs, operating home-based practices in 14 states. The role focuses on annual risk assessments conducted in residential settings or via telehealth under a physician-driven model. The organization focuses on delivering personalized healthcare to patients who face access barriers.

Responsibilities

• Conduct comprehensive health risk assessments in residential settings or via telehealth to identify active and chronic disease conditions along with physical, mental, and social needs
• Take patient history, perform examinations, and determine diagnoses
• Provide written documentation of patient visits according to NCQA standards
• Measure and record patient vital signs as necessary
• Place case management referrals and communicate with primary care providers as needed
• Communicate with patients, caregivers, agency nurses, other providers, and vendors to support diagnosis and care
• Perform all clinical duties while observing OSHA Universal Precautions
• Maintain patient confidentiality
• Attend required meetings and in-services and participate in committees as requested
• Participate in professional development activities and maintain professional licenses and affiliations

Requirements

• Active and unrestricted nurse practitioner license to practice in coverage states
• Board certification through American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA)
• Active BLS Certification
• Current enrollment in Medicare/Medicaid
• Valid driver’s license and good driving record
• EHR skills
• Geriatric training or experience
• Adaptability to differing weather conditions and patients’ home or residential environments
• Full range of body motion including handling and lifting patients, manual and finger dexterity, eye-hand coordination, normal visual acuity, normal hearing, standing, bending, walking, and stair climbing
• Regular lifting and carrying of items weighing up to 50 pounds
• Ability to ride in an automobile or van up to 150 miles daily in urban or rural settings and ability to drive if necessary
