Nurse Practitioner (NP) Primary Care ($20k Retention)

HarmonyCares is hiring a Nurse Practitioner for a primary care role based in Williamsburg, Virginia. The position provides in-home primary care services to patients with complex healthcare needs under a national value-based model that operates across 14 states with a team-based, physician-driven approach. The schedule runs Monday through Friday with no hospital rounds or holidays required.

Responsibilities

• Provide primary care to high-acuity patients during in-home visits
• Deliver patient-centered care with focus on individual needs rather than visit volume
• Operate within an integrated, team-based model that includes physician oversight
• Maintain minimal call responsibilities

Requirements

• Active Nurse Practitioner license in Virginia
• Federal DEA license or eligibility
• Board certification
• Active CPR/BLS certification
• Valid driver’s license with good driving record
• Geriatric training or experience preferred
