Nurse Practitioner Primary Care- Queens NY $20k Retention Bonus

Nurse Practitioner Primary Care role with HarmonyCares in Whitestone, New York. HarmonyCares delivers value-based, in-home primary care to patients with complex healthcare needs across multiple states through a team-based, physician-driven model. The position follows a Monday-through-Friday schedule with no hospital rounds or holidays.

Responsibilities

• Provide primary care to high-acuity patients in their homes Monday through Friday

Requirements

• Licensed as a Nurse Practitioner in New York
• Federal DEA License or eligibility
• Board Certification
• Active CPR/BLS Certification
• Geriatric training or experience preferred
• Valid driver’s license and good driving record
