The Physician Leader role at HarmonyCares is located in Williamsburg, Virginia. HarmonyCares delivers in-home primary care services for patients with complex healthcare needs under a team-based, physician-driven model. This position functions as the clinical leader for an assigned pod, maintains a dyad partnership with the Market Manager, collaborates with multiple clinical team members, and carries no hospital rounds or weekend/holiday requirements.
Responsibilities
Act as on-site supervising physician for assigned advanced practice providers and physicians within the pod, ensuring the total number of collaborative practice agreements does not exceed state regulations
Participate in at least one ride-along with each collaborative advanced practice provider per quarter and provide complex patient visits as needed
Collaborate with community health workers, medical social workers, nurse navigators, care managers, patient care coordinators, and other clinical team members
Lead high-risk huddles and ensure documented plans of care that include all care management team members
Conduct 10 chart audits per month or the number required by state regulations, whichever is higher
Perform routine panel reviews with advanced practice providers and sign DME, death certificates, and other orders as needed
Review quality metrics and pod-level performance to identify improvements for HEDIS outcomes
Review clinician capacity and productivity, then partner with site medical directors to implement countermeasures when providers are off track
Assist pod providers with EMR efficiency and participate in patient care for the assigned panel, including high-risk or complex cases and cognitive assessments
Lead clinical education on accurate diagnosis, documentation, and management of chronic conditions
Handle discipline actions, performance reviews for direct reports, compliance matters, and HR resolutions
Assist with service inquiries and resolution of patient complaints for the pod as needed
Requirements
Bachelor’s degree
Doctor of Medicine or Osteopathy
Completion of an accredited residency program
Unrestricted medical license
Unrestricted DEA registration
Active CPR certification
Valid driver’s license with a good driving record
Board certification required for all newly hired or promoted physicians on or after January 1, 2026, and must be maintained in accordance with organizational and regulatory standards
Three years of experience as a physician collaborating with advanced practice providers (preferred)
Experience in geriatrics or with complex patients (preferred)
