Job Description

HarmonyCares is hiring a Physician Leader, Primary Care for its home-based primary care practice in Virginia Beach, Virginia. The full-time Monday through Friday role provides clinical oversight for 4-5 advanced practice providers and divides time between 2.5 days of administrative and oversight duties and 2.5 days of direct patient care for a panel of 140 patients. The position operates in a value-based care model serving frail Medicare patients in their homes and includes base compensation plus quality bonus totaling $300k or more along with a $50k retention bonus. The organization emphasizes patient-centered care delivered in the home setting.

Responsibilities

  • Provide on-site clinical supervision for assigned advanced practice providers and physicians within the pod while maintaining collaborative practice agreements within state limits
  • Conduct at least one ride-along observation with each collaborative advanced practice provider per quarter
  • Complete complex patient visits as needed for supervised advanced practice providers
  • Collaborate with pod clinical team members including community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators
  • Support clinical development of advanced practice providers through evidence-based care practices
  • Lead high-risk patient huddles and ensure documented care plans involving all relevant team members
  • Perform 10 chart audits per month or the number required by state regulations, whichever is greater
  • Review quality metrics and pod performance data to identify improvements in HEDIS measures
  • Monitor clinician capacity and productivity and partner with site medical directors on corrective actions
  • Coordinate with the regional medical director on care improvement opportunities
  • Handle service inquiries, patient complaints, and related requests for the pod
  • Assist pod providers with EMR efficiency
  • Deliver direct patient care to the assigned panel and additional high-risk or complex cases as needed
  • Lead clinical education on diagnosis, documentation, and chronic condition management
  • Support compliance and human resources resolutions including discipline actions and performance reviews for direct reports
  • Sign DME orders, death certificates, and other documents as required by state regulations on behalf of advanced practice providers

Requirements

  • Bachelor’s degree
  • Doctor of Medicine or Doctor of Osteopathy
  • Active medical license
  • Active CPR certification
  • Valid driver’s license with acceptable driving record
  • Board certification preferred
  • One year of experience at HarmonyCares Medical Group preferred
