Job Description

HarmonyCares operates a value-based in-home primary care practice serving patients with complex healthcare needs in Youngstown, Ohio. The Medical Pod Leader functions as the clinical lead for a pod team that includes advanced practice providers and support staff, while maintaining a dyad partnership with the Market Manager. The position requires participation in patient visits, chart audits, quality reviews, and supervision of collaborative practice agreements, with no holidays, weekends, or hospital rounds.

Responsibilities

  • Supervise assigned advanced practice providers and physicians as the on-site collaborating physician while staying within state limits on collaborative agreements
  • Complete at least one ride-along observation with each collaborative advanced practice provider per quarter
  • Provide complex patient visits as needed for supervised advanced practice providers
  • Collaborate with community health workers, medical social workers, nurse navigators, care managers, and patient care coordinators
  • Develop advanced practice provider clinical skills through evidenced-based care training and professionalism guidance
  • Lead high-risk patient huddles and ensure documented care plans that include all care management team members
  • Conduct ten chart audits per month or the number required by state regulations, whichever is greater
  • Review quality metrics and pod-level performance data to identify and implement improvements in HEDIS outcomes
  • Monitor clinician capacity and productivity; develop and support countermeasures when performance falls below targets
  • Partner with the Regional Medical Director on care improvement opportunities
  • Assist with service inquiries, patient complaint resolution, and difficult patient or family conversations
  • Support pod providers in achieving greater EMR efficiency
  • Deliver direct patient care for assigned panels and high-risk or complex cases, including cognitive assessments and physician face-to-face requirements
  • Lead clinical education on accurate diagnosis, documentation, and chronic condition management
  • Handle compliance matters, HR resolutions, discipline actions, and performance reviews for direct reports
  • Sign DME orders, death certificates, and other documents as required by state regulations on behalf of advanced practice providers

Requirements

  • Bachelor’s degree
  • Doctor of Medicine or Doctor of Osteopathy
  • Active medical license
  • Active CPR certification
  • Valid driver’s license with acceptable driving record
