LVN LPN

Full-time LPN position on 2nd shift at Belmont Village Senior Living in Chicago, Illinois. The role supports an Assisted Living and Memory Care community, with weekend availability required and an additional $3 per hour for 2nd shift plus extra pay for select weekend shifts. Free on-site garage parking is provided.

Responsibilities

• Follow up on physician orders, coordinate services with ancillary providers, and report changes in resident condition or other clinical information to physicians and families.
• Administer medications to an assigned group of residents according to physician orders and document any adverse reactions or observations.
• Oversee Medication Technicians and care providers to ensure resident care and service plans are followed.
• Make situational decisions and address conflicts constructively.
• Update and review resident medical records, clinical documentation, and incident reports.

Requirements

• Active LVN or LPN license in Illinois.
• CPR certification or ability to obtain within 30 days of hire.
• One year of medication administration experience preferred.
• Intermediate computer and mobile device skills.
