LVN LPN

LVN and LPN positions are open at Belmont Village Senior Living in Oak Park, Illinois. The roles operate in an Assisted Living and Memory Care community and cover full-time second shift, part-time first shift, part-time first shift with rotating weekends, and PRN/on-call schedules. Duties center on medication administration, clinical coordination, and oversight of care staff.

Responsibilities

• Follow up on physician orders, coordinate services with ancillary providers, and report changes in resident condition to physicians and families.
• Administer medications to an assigned group of residents according to physician orders and document any adverse reactions or clinical observations.
• Oversee Medication Technicians and care providers to ensure resident care and service plans are followed.
• Update and review resident medical records, clinical documentation, and incident reports.

Requirements

• Active LVN or LPN license in Illinois
• CPR certification or ability to obtain within 30 days of hire
• One year of medication administration experience preferred
• Intermediate computer and mobile device skills
