Medical Assistant (Clinical Partner)

The Medical Assistant (Clinical Partner) role at HarmonyCares supports in-home primary care services for patients with complex healthcare needs. HarmonyCares operates home-based primary care practices across 14 states from its headquarters in Troy, Michigan, under an integrated team-based model. The position follows daytime hours with no weekends or holidays and involves travel to residential settings with manageable daily visit volumes.

Responsibilities

• Assist primary care physicians, nurse practitioners, and clinical teams with delivery of primary health care and patient care management in residential settings.
• Coordinate daily travel itineraries and transport clinicians to scheduled appointments.
• Prepare patients for examination and treatment, including minor procedures when required.
• Secure patient consent forms and record patient care documentation accurately and promptly.
• Coordinate patient care according to clinician direction, company standards, and policies.
• Perform phlebotomy via venipuncture, EKG, and other procedures as requested by clinicians.
• Label, store, and package specimens properly for laboratory delivery.
• Identify and assist with closure of quality gaps.
• Document and update patient care plans, Social Determinants of Health assessments, and other evaluations in the electronic medical record.
• Maintain compliance with CLIA, OSHA, and HIPAA guidelines.
• Prepare and maintain supplies and equipment for treatment, including supply inventory, medical bag, and vehicle.
• Ensure company vehicle cleanliness and adherence to periodic maintenance schedules.
• Attend required meetings and in-services.

Requirements

• High school diploma or GED equivalent.
• Graduation from an accredited Medical Assistant, Phlebotomy, EMT, or related medical training program, or one year of experience as a Medical Assistant or Phlebotomist.
• Current CPR certification meeting company guidelines.
• Knowledge of medical terminology.
• Basic computer literacy including EMR, iPad, and GPS use.
• Valid driver’s license with a good driving record.
• Ability to perform full range of body motion including handling and lifting patients.
• Capability to drive up to 150 miles daily in urban or rural settings.
• Ability to lift and carry items weighing up to 50 pounds.
• Ability to read and understand road maps.
• Capacity to sit or stand for extended periods.
• Certified Medical Assistant credential.
• Six months of patient care experience, preferably in geriatric settings.
• Strong phlebotomy skills.
• Ability to obtain Medical Assistant certification within 180 days of hire.
