Medical Assistant (Clinical Partner)

This position is for a Medical Assistant (Clinical Partner) with HarmonyCares, a national value-based provider of in-home primary care services for patients with complex healthcare needs. The role operates in residential settings in Grand Rapids, Michigan, supporting primary care delivery through an integrated team-based model with daytime hours and no weekends or holidays. Responsibilities

• Coordinate daily travel itinerary and transport clinicians to scheduled appointments
• Prepare patients for examination, treatment, and minor procedures
• Secure patient consent forms and record care documentation accurately and timely in the electronic medical record
• Coordinate patient care as directed by clinicians according to company standards and policies
• Perform phlebotomy via venipuncture, EKG, and other procedures as requested by clinicians
• Label, store, and package specimens properly for laboratory delivery
• Assist in identifying and closing quality gaps
• Document and update patient care plans, Social Determinants of Health assessments, and other evaluations in the patient chart or electronic medical record
• Maintain compliance with CLIA, OSHA, and HIPAA guidelines
• Prepare and maintain supplies and equipment for treatment, including supply inventory, medical bag, and vehicle where applicable
• Ensure company vehicle cleanliness and adherence to periodic maintenance schedules where applicable
• Attend required meetings and in-services

Requirements

• High School graduate or GED equivalent
• Graduate of an accredited Medical Assistant, Phlebotomy, EMT, or related medical education/training program, or 1 year of experience as a Medical Assistant or Phlebotomist
• Current CPR certification that meets company guidelines
• Knowledge of medical terminology
• Basic computer literacy including EMR, iPad, and GPS experience
• Valid driver’s license and good driving record
• Ability to perform full range of body motion including handling and lifting patients
• Ability to drive up to 150 miles daily in urban and/or rural settings
• Ability to lift and carry items weighing up to 50 lb
• Ability to read and understand road maps
• Ability to sit or stand for long periods of time
