Medical Assistant(Clinical Partner)

The Clinical Partner (Medical Assistant) role is with HarmonyCares, a national provider of in-home primary care for patients with complex healthcare needs, based in Bay City, Michigan. The position supports physicians and nurse practitioners delivering care in residential settings under a team-based model and follows daytime hours with no weekends or holidays. Daily travel to patient homes is required, with manageable visit volumes.

Responsibilities

• Coordinate daily travel routes and transport clinicians to patient appointments in residential settings
• Prepare patients for examinations, treatments, and minor procedures
• Obtain patient consent forms and document care in the electronic medical record in a timely manner
• Perform phlebotomy via venipuncture, EKGs, and other procedures as directed by clinicians
• Label, store, and package specimens for laboratory delivery
• Update patient care plans, Social Determinants of Health assessments, and other documentation in the EMR
• Maintain compliance with CLIA, OSHA, and HIPAA guidelines
• Manage supply inventory, prepare medical bags and vehicles, and ensure company vehicles receive scheduled maintenance
• Attend required meetings and training sessions

Requirements

• High school diploma or GED equivalent
• Completion of an accredited Medical Assistant, Phlebotomy, or EMT program, or one year of experience as a Medical Assistant or Phlebotomist
• Current CPR certification meeting company standards
• Knowledge of medical terminology
• Basic computer literacy with EMR, iPad, and GPS systems
• Valid driver’s license and acceptable driving record
• Ability to perform full range of body motion including patient handling and transfers
• Ability to drive up to 150 miles daily in urban or rural areas
• Ability to lift and carry items up to 50 pounds
• Ability to read and interpret road maps
• Capacity to sit or stand for extended periods
