Our focus is on providing comfort, not a cure , and quality of life, not duration As a Home Health Aide on our H ospice team , you will care for patients where they need you most, whether at home, a nursing facility , assisted living center or other location.
, you can help enhance their outlook on their experience, even when their futures seem uncertain. Every day, you can make a lasting difference in both the lives of your patients and their loved ones. A caregiver in this position works nights, 9:00pm - 5:00am. This would be 3 days a week, 8 hour shifts. Every 3rd weekend at the Hospice Care Center. This role will support our Southern Territory and the Hospice Care Center. A caregiver who excels in this role will: Perform basic patient care activities. Assist with procedures ordered by physician , with supervis ion from an RN, PT, OT or ST as appropriate . Assist patients with tending to personal care, activities of daily living and transfers/transport. Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff. Assist with keeping the patients environment clean and orderly. Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. Escalate change in patient condition to patient’s physician as appropriate .
for the ideal future caregiver include: High S chool D iploma or GED State Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse’s Aide Registry One year of clinical experience as an A ide in an acute, rehab or skilled nursing setting Valid D river’s L icense (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage Basic Life Support (BLS) Certification through the American Heart Association (AHA) within orientation period Preferred qualifications for the ideal future caregiver include: Hospice and Palliative Care Nurse Assistant (CHPNA) C ertification Home C are experience Our caregivers continue to create the best outcomes for our patients across each of our facilities

Physical Demand requirements are in excess of those for Light Work Personal Protective Equipment: Follows standard precautions using personal protective equipment as required
