What Makes Home Care Different? As an RN on our Home Care team, you will: • Lead and coordinate individualized patient care plans • Deliver 1:1 skilled nursing care in the home • Collaborate with interdisciplinary clinicians • Provide advanced clinical treatments and patient education • Help prevent hospital readmissions and improve long-term outcomes You won’t just treat patients — you’ll guide their recovery, empower families, and make a meaningful difference in their daily lives. Weekend RN Opportunity We are seeking a part-time Registered Nurse to join our Weekend Team, providing care South Shore territory. Schedule: • Part time (20 hours) - Benefit eligible • Every Saturday & Sunday • 8:00 AM – 6:30 PM Your schedule includes dedicated time for documentation, chart review, and care coordination, allowing you to focus on delivering thoughtful, patient-centered care. The RN Role in Home Care Our Registered Nurses oversee and coordinate the full plan of care while collaborating closely with LPNs, therapists, and social workers to ensure patients receive comprehensive, high-quality care at home. You’ll typically complete starts of care, resumptions of care, evaluations, or revisits.
  • Provide quality, clinical services to patients in various settings in the community. Patient care is delivered within organizational philosophy, policy and standards of community health practice. The admissions process and case management responsibilities are performed in accordance with MGB Healthcare at Home policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications. Skilled nursing care is provided in the form of assessment, teaching, treatment, documentation and care coordination demonstrating high quality customer service, and financial awareness.

Qualifications

• Graduate of an approved School of Nursing • Current Massachusetts license as Registered Nurse • Minimum of one year previous nursing experience in acute care setting

Prior home care experience preferred • Travel required within MGB Healthcare at Home geographic area • Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred • Strong interpersonal and customer service and ability to work with a diverse population • Ability to communicate effectively in writing, verbally and electronically • Ability to work as a member of an interdisciplinary team of health care providers • Ability to work independently, be self-directed and adapt to unpredictable circumstances • Ability to organize and prioritize work, and adapt to changing situations Additional Job Details (if applicable) Who Thrives Here?
