RN Intake Hospital Liaison

Job Description

The RN Intake Hospital Liaison is a Registered Professional Nurse responsible for coordinating the intake process for patients referred to home care services. Serving as the primary liaison between hospitals, referral sources, patients, families, and internal teams, this role ensures a seamless transition from hospital to home.

Responsibilities

• include reviewing referrals, coordinating appropriate home care services, facilitating timely admissions, and providing exceptional customer service by addressing patient inquiries and concerns with professionalism, compassion, and efficiency. Requirements

Current NYS RN License Allscripts a MUST CHHA experience a MUST Computer literacy; experience with Home Care Home base a MUST Excellent communication skills Highly organized, possess a strong attention to detail Experience with multitasking and working in a high pressure work environment
