The Breast Cancer Nurse Navigator serves as a clinical expert, patient advocate, educator, and care coordinator for patients with breast cancer and high-risk breast conditions

The navigator provides individualized support throughout the continuum of care, from diagnosis through treatment, survivorship, and follow-up Working collaboratively with physicians and multidisciplinary team members, the navigator facilitates timely access to care, addresses barriers to treatment, coordinates services, and promotes a seamless patient experience.

Education

Required - Registered Nurse Diploma Preferred - Associates or Bachelors degree in Nursing Work Experience Required - 2 years Nursing experience Preferred - Experience in o ncology, breast health, infusion, surgical oncology, radiation oncology, or navigation Certifications Required - Current RN License in the state of practice Basic Life Support (BLS) from the American Heart Association Preferred - Certification in clinical specialty area Knowledge Skills and Abilities (KSAs) Must have computer skills and dexterity required for data entry and retrieval of patient information

Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process

Must be proficient with Windows-style applications, various software packages specific to role and keyboard Excellent conflict resolution skills Job Duties Serves as the primary clinical navigator, educator, and patient advocate for individuals with breast cancer throughout the continuum of care

Develops therapeutic relationships with patients and caregivers, conducts nursing assessments, identifies barriers to care, and provides individualized education regarding diagnosis, treatment options, survivorship, genetic testing, and supportive care resources

Coordinates multidisciplinary care among surgical oncology, medical oncology, radiation oncology, radiology, pathology, genetics, rehabilitation, and supportive care services

Facilitates timely scheduling of appointments, diagnostic testing, procedures, consultations, and treatments to minimize delays in care Assists with genetic risk assessment processes and coordinates referrals for genetic counseling and testing when appropriate

Triages patient concerns and collaborates with providers to address symptom management, treatment-related concerns, and care plan changes Maintains accurate and timely documentation within the electronic medical record, including navigation assessments, interventions, barriers, acuity evaluations, and outcomes

Collaborates with physicians and multidisciplinary teams through tumor boards, care planning conferences, and ongoing coordination activities to ensure optimal patient outcomes

Facilitates referrals to supportive care services, including social work, nutrition, psycho-oncology, rehabilitation, financial counseling, survivorship resources, and community programs

Participates in community outreach, education, support programs, accreditation activities, quality improvement initiatives, and professional development opportunities

Practices in accordance with Oncology Nursing Society (ONS) Oncology Nurse Navigator Core Competencies and Association of Oncology Nurse Navigators (AONN) Navigation Standards

Work Environment

/ Scheduling This position follows a hybrid work model Remote work eligibility is based on departmental needs, performance expectations, and operational requirements

Hybrid work arrangements may be available following successful completion of orientation and demonstration of independent role competency Employees in this role may work remotely up to one (1) day per week, with the remaining workdays performed on-site

The schedule may be modified based on patient care needs, staffing requirements, and organizational priorities Physical and Environmental Demands The physical essential functions of this job include (but are not limited to) the following: Frequently exerting 10 to 20 pounds of force to move objects; occasionally exerting up to 100 pounds of force
