• Summary The Case Management Team Lead reports to the Director of Case Management and/or Manager of Case Management. Under general supervision, the Case Management Team Lead provides professional supervision and direction to care team members in the Case Management Department (RNs, MSWs, Program Coordinators) to achieve the best clinical outcomes for patient groups. This position serves as a clinical expert and resource related to Case Management, discharge planning, and care transitions. Minimum Training, Education and Licensure : Bachelors degree in Nursing and two years’ case management work experience required; Master’s Degree Preferred. Licensure as a registered nurse by the state of South Carolina or compact state required. Case Management Society of America(CSMA) or Americian Case Management Association(ACMA) certification preferred. Must obtain certification within 1 year, if not currently certified. OR Master’s Degree in Social Work and licensure by South Carolina LLC; Supervisory experience preferred. Case Management Society of America(CSMA) or Americian Case Management Association(ACMA) certification preferred. Must obtain certification within 1 year, if not currently certified. For every 100 additional pounds, assistance will be required from another healthcare worker. 20 lbs. of force is needed to push a 400 lb. patient in a wheelchair on carpet. 25 lbs. of force is required to push a stretcher with a patient with one hand. Ability to maintain 20/40 vision, corrected, in one eye or with both eyes. (Continuous) Ability to see and recognize objects close at hand or at a distance. (Continuous) Ability to match or discriminate between colors. (Continuous) Ability to determine distance/relationship between objects; depth perception. (Continuous) Good peripheral vision capabilities. (Continuous) Ability to maintain hearing acuity, with correction.
