Job Description

We are seeking a highly skilled and compassionate Registered Nurse (RN) in the Augusta, GA/Aiken, South Carolina area to join our team for assignments across the state of Georgia and South Carolina

This role allows you to stay close to home while gaining diverse experience, making a meaningful impact on patients' lives and contributing to different communities within the state.

As a Resource Nurse, you will travel 2–3 days per week to support a variety of clinical responsibilities This role is responsible for completing assessments for new, transferred, and current patients, as well as reviewing, developing, and coordinating individualized plans of care

You will provide education to patients, their families, and home health aides regarding medical conditions, disease progression, and care needs The Resource Nurse will also help with bridging staffing gaps.

The Resource Nurse will also support the onboarding and training of contracted caregivers. In addition, this position provides clinical support to the Director of Nursing, Compliance Manager, and the broader care team.

  • Strong collaboration with patients, families, skilled and non-skilled caregivers, and interdisciplinary team members is essential to ensure effective care planning, service utilization, and overall patient outcomes. This role also includes connecting patients with appropriate community resources and recommendations.

This role is a Full-Time benefit eligible opportunity, with care being provided at the patient’s residence. Patients range from high acuity to those who only need basic medical management.

Shift: Full-Time, Day Shift (8:00 AM - 5:00 PM, Monday through Friday).
