Job Description

  • Department: 34000 Milwaukee South - Administration: Nursing Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Full Time Home Health Float Pool position based in WI supporting the southern patient service markets. Areas of work based on assignment. Typical schedule is Monday through Friday 8:00am to 4:30pm with some variability based on departmental needs. Every 4th weekend rotation required. Pay Range: $35.50 - $53.25 Major Responsibilities Complete scheduled patient visits and collaborate with the interdisciplinary team. Conduct comprehensive patient assessments and update plans of care as needed.
  • Communicate treatment changes with physicians and the primary registered nurse. Monitor patient response to care and revise interventions accordingly. Coordinate scheduling and case conferences with primary nurse when appropriate. Assists with the discharge planning process according to agency standards and utilizes interagency/system and community resources to ensure continuity of care after discharge. Communicates problems to physicians and primary Registered Nurse, and clearly states plans, actions and goals so that changes are reflected as they occur. Maintain accurate documentation in compliance with all regulations. Follow agency policies and safely perform required patient care activities. Provide age-appropriate care based on patient needs and developmental stage. Minimum Job Requirements Education ​ Associate’s degree in nursing. Certification / Registration / License Active RN license in state of practice. Basic Life Support (BLS) – American Heart Association Valid drivers license and reliable transportation. Experience 3-4 years clinical experience. 1-2 years in home health. Knowledge / Skills / Abilities ​​Ability to work independently and make sound clinical decisions Excellent communication (written and verbal) and interpersonal skills. Ability to develop rapport and maintain positive, professional relationships with a variety of patients, staff and physicians. Cultural competence and respect for patient diversity. Good organization, prioritization and problem-solving skills.

Physical Requirements

and Working Conditions Ability to multi-task with frequent interruptions. Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
