 Department Details Join a small team making a great impact on our community This team sees chronic and complex patients in between their PCP visits, essentially a bridge to keep patients out of the hospital
Patients are cared for at both locations of Active Generations in Sioux Falls, in Canton, and in Worthington Summary The Registered Nurse (RN) works as an integral member of the inter-professional collaborative practice team administering community-based strategies aimed at improving the health of the community through education, health screening and coordinating evidence-based programming.
Responsible for utilizing the nursing process to provide nursing care to assigned area including shelters and non-structured community based settings across the continuum of care
Assists individuals in accessing specialized programs through the use of screenings and referrals Considers evidence-based outcomes when selecting interventions Depending on location and home health needs, may perform as a respite care nurse
Maintains relationships with the local and regional community, including churches and other service organizations, to promote continued support and capacity building
May assist in organizing and performing community flu shot clinics based on needs of the population Identifies, strengthens and links with the community's tangible resources, including schools, businesses, local service groups, and intangible resources such as community spirit
Recruits, trains and retains community volunteers Demonstrate critical thinking skills to analyze data and appropriately respond to changing or difficult situations
The nursing process is utilized to assess, diagnose, establish a patient centered plan with defined outcome criteria; this plan includes interventions and means of evaluating the human responses to actual or potential health problems
Assumes the responsibility for the outcome of nursing care during a specified work period when they are present in the community setting This practice includes dependent, interdependent and independent functions
Practices within the full scope of nursing expertise with knowledge of and utilizing appropriate age related nursing standards.
within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.
Bachelor’s degree in nursing preferred Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA)
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state
Obtains and subsequently maintains required department specific competencies and certifications Additional training and certification may be required for area of focus within first year of employment, including but not limited to, Foot and Nail Care Education Course with a clinical component and demonstrate competence in foot care skills and/or be a Certified Foot and Nail Care Nurse, or Foundations in Faith Community Nursing course
