Work location

is flexible with opportunities to go into the community, home, and clinic setting with clients The role of a RN on the ACT team is to provide comprehensive, client-centered nursing care and support to individuals with complex behavioral health needs

The RN collaborates with clients, local pharmacies, healthcare providers, and other members of the treatment team to promote medication adherence, coordinate medical care, and assist clients in navigating the healthcare system

This role is integral to ensuring continuity of care, promoting overall health and wellness, and delivering high-quality nursing services within a community-based setting

The RNs on the ACT team participate in a team-nursing model, working collaboratively with other RNs to share responsibilities, support one another, and ensure clients receive coordinated and comprehensive care including medications drops, administration of injections, and completing various mental health assessments as needed

Team members have access to company fleet vehicles, work cell phones, licenses renewal reimbursement, dollars and training days for CEUs, peer led support groups and frequent potlucks/shared meals

We are highly collaborative and meet daily to review our caseload The RN role does have a required on-call component for after hours and weekends, additional compensation for this time

Call rotation is typically one week every 4-6 weeks depending on preference Apply to learn more about our awesome team! Summary Responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care

Collects and relays information (assessment, screening, diagnostic testing results and risk stratification) to providers on both hospital and clinic patients enrolled in the approved programs.

Job Description

Develops and provides individualized patient education based on assessed needs Develops and implements individualized care plan Assists with the coordination of patient care across the continuum by collaborating with the inter-professional team and community partners

Works with the clinical team and may manage the disease registries by identifying and following up with patients not meeting clinical and evidenced-based goals as well as Sanford approved clinical guidelines

Provides services that focus on health and wellness Provides healthcare leadership, coordination of services, and the implementation of evidence-based practice to support the overall goals of the designated setting and/or program

Applies population health care management strategies to panels of patients Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting.

Qualifications

Bachelors degree in Nursing preferred or Associates degree in Nursing required Graduate from an accredited nursing program required Currently holds an unencumbered RN license with the Minnesota State Board of Nursing
