Assists the physicians/registered nurses in the care of clients in their homes through provision of comprehensive skilled nursing services directed toward pain and symptom management and end of life teaching/support Implements and evaluates plan of care, treatments and procedures that contribute to the physiological and psychosocial outcomes of the patient
Collaborates with the primary RN in providing quality patient care needs as established in the Plan of Care Provides services according to agency policies and supplements the nursing care needs of the client as provided by the RN
Assists the RN or physician in performing specialized procedures and duties Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs
Actively participates in multi-disciplinary team meetings as assigned Prepares clinical and progress notes documenting outcomes of interventions Accurately completes all required clinical documentation consistently per RHC standards which includes completing and "locking" patients notes and verbal orders within 24 hours
Provides comprehensive skilled nursing services to assigned patients in case load Remains fully cognizant of Medicare, Medicaid, and Commercial insurance requirements, along with scope of service.
