<li>Provide skilled nursing care to patients in their homes under the supervision of an RN or physician.</li>

<li>Monitor patient health status and report changes in condition.</li>

<li>Administer medications and treatments as prescribed.</li>

<li>Perform wound care, catheter care, and other clinical procedures.</li>

<li>Educate patients and families on disease management and care plans.</li>

<li>Document patient visits and maintain accurate medical records.</li>

<li>Collaborate with interdisciplinary team to ensure quality patient care.</li>

</ul>

<li>Graduate of an accredited practical nurse or vocational nursing program (LVN/LPN).</li>

<li>Minimum 1 year of experience, preferably in home care or med/surg.</li>

<li>Current state license and CPR/BLS certification.</li>

<li>Strong communication and problem-solving skills.</li>

<li>Ability to meet physical demands of the job.</li>

<li>Valid drivers license and a reliable, insured vehicle.</li>
