<p><span data-contrast=&quot;none&quot;>Ennoble Care is hiring for a Full-time Hospice Registered Nurse Case Manager to work within and around the Fulton / Cobb County, GA region

As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.</span><span data-ccp-props=&quot;{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}&quot;>&nbsp;</span></p>

<p><span data-contrast=&quot;none&quot;>Responsible for identifying patient and loved ones/caregiver needs and for providing supportive care in accordance with the attending physician’s orders and plan of care and the policies and procedures. The functions of this position will include the following:</span></p>

<li><span data-contrast=&quot;none&quot;>Shall be assigned patient caseload by the Patient Care Coordinator or designee</span></li>

<li><span data-contrast=&quot;none&quot;> Assume primary responsibility for a patient caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process</span></li>

<li><span data-contrast=&quot;none&quot;> Initiate communication with attending physicians, Hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient and loved ones/caregivers</span></li>

<li><span data-contrast=&quot;none&quot;> Maintain regular communication with the attending physician concerning patient and loved ones/caregivers</span></li>

<li><span data-contrast=&quot;none&quot;> Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support</span></li>

<li><span data-contrast=&quot;none&quot;> Provide accurate and timely documentation in electronic medical records.</span></li>

<li><span data-contrast=&quot;none&quot;> Participates in and contributes appropriate information for Interdisciplinary Group meetings</span></li>

<li><span data-contrast=&quot;none&quot;> Accept responsibility for coordinating physical care of the patient by teaching primary caregivers, volunteers, and employed caregivers or by providing direct care as appropriate</span></li>

<li><span data-contrast=&quot;none&quot;> Meet regularly with the Hospice nursing staff to review problems or unique issues from caseloads; share professional support; and exchange feedback aimed toward enhancing professional growth</span></li>

<li><span data-contrast=&quot;none&quot;> Attend patient care conferences and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation</span></li>

<li><span data-contrast=&quot;none&quot;> Supervise and document the NAC every two weeks or as assigned</span></li>

<li><span data-contrast=&quot;none&quot;> Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues</span></li>

<li><span data-contrast=&quot;none&quot;> Assume the role of associate nurse when responding to patient/loved ones/caregivers of other primary nurses during off-hour times or as requested by the Patient Care Coordinator to fill patient/family needs during the absence of other primary nurses</span></li>

<li><span data-contrast=&quot;none&quot;> Provide appropriate support at time of death and period of bereavement</span></li>

<li><span data-contrast=&quot;none&quot;> Participate in the Hospice orientation and in-service training programs for professional staff</span></li>

<li><span data-contrast=&quot;none&quot;> Participate in agency and community programs as required to promote professional growth and understanding of hospice care</span></li>

<li><span data-contrast=&quot;none&quot;> Maintain familiarity with policies of the agency and rules and regulations of State and Federal bodies which govern hospice</span></li>

<li><span data-contrast=&quot;none&quot;> Accept other assignments as appropriate</span><strong>&nbsp;</strong>&nbsp;</li>

<p><strong><span data-contrast=&quot;none&quot;>Qualifications:</span></strong>&nbsp;</p>

<ul>

<li>Must be a graduate from an accredited School of Nursing.</li>

<li>Must be a Registered Nurse who is currently licensed to practice nursing under the provisions of&nbsp;<br>Article 1 of Chapter 26 of Title 43 of the Official code of Georgia.</li>

<li>One year of RN experience. Hospice experience preferred.</li>

<li>Must have excellent assessment skills and problem-solving skills.&nbsp;</li>

<li>CPR certified for patient care.</li>

<li>Should be skillful in organization and in the principles of time management and have knowledge&nbsp;<br>of case management processes.&nbsp;</li>

<li>Must be a licensed driver with an insured automobile in good working order.</li>

<li>The ability to communicate well, both verbally and in writing.&nbsp;</li>

<li>Must have no exceptions to RN license, and be able to administer pharmaceuticals, perform&nbsp;<br>medication reconciliation, and any nursing procedures or special treatments needed within the<br>scope of nursing practice for state

All skills must be deemed competent before actual hands-on patient care, and when procedures or treatments are not usually performed.</li>
