The Cadence Health team is seeking a Registered Nurse Coordinator to monitor, assess, and triage patients based on vital signs and clinical data

This role supports the ongoing management of patient treatment plans and medications through close collaboration with Cadence Nurse Practitioners and each patient’s physician or provider, ensuring timely, coordinated, and high-quality care.

What youll do:

<ul>

<li style="color: rgb(0, 0, 0);"> Monitor patient vitals, symptoms, and labs to identify patients in need of clinical interventions. </li>

<li style="color: rgb(0, 0, 0);"> Follow up with patients with abnormal readings to gather more information on their clinical status and triage appropriately. </li>

<li style="color: rgb(0, 0, 0);"> Respond to inbound patient clinical questions and escalations by phone and text message. </li>

<li style="color: rgb(0, 0, 0);"> Lead virtual follow-ups with patients to support program enrollment, treatment plan changes, medication adherence, and achievement of lifestyle goals. </li>

<li style="color: rgb(0, 0, 0);"> Support clinical documentation and development of care summaries for patients physicians.</li>

<li style="color: rgb(0, 0, 0);">Be in charge of making sure every healthcare interaction with Cadence is an exceptional experience that prioritizes the well-being of the patient and aligns with the goals of the health system.</li>

<li style="color: rgb(0, 0, 0);">Support internal growth efforts to help Cadence scale exceptional care delivery to patients with CHF, hypertension, and other chronic conditions like type 2 diabetes.</li>

<li style="color: rgb(0, 0, 0);">Be instrumental in shaping the culture of one of the fastest-growing teams at Cadence. </li>

What you need:

<ul>

<li style="color: rgb(0, 0, 0);">4+ years experience treating CHF, hypertension, and Diabetes patients either in an outpatient or inpatient setting. </li>

<li style="color: rgb(0, 0, 0);"> Compact multi-state licensure (RN compact).</li>

<li style="color: rgb(0, 0, 0);"> Experience working in a CHF bridge clinic environment. </li>

<li style="color: rgb(0, 0, 0);"> Experience working with remote patient monitoring technology. </li>

<li style="color: rgb(0, 0, 0);"> Passion for the patient/ customer experience and systematically improving healthcare with digital innovation. </li>

<li style="color: rgb(0, 0, 0);"> Independent thinker/ operator. </li>

<li style="color: rgb(0, 0, 0);"> Ability to monitor patient vitals, symptoms and labs to identify patients in need of clinical interventions. </li>

The anticipated compensation range for this role is $45 to $50 per hour, which is based on role scope, level, and location
