<p><strong><em>As the Aide, you will:&nbsp;</em></strong></p>

<li>Work in collaboration with the RN to fulfill the defined patient-specific Aide care plan&nbsp;</li>

</ul>

<ul>

<li>Collaborate with the care team as an extension of nursing or therapy services to ensure all patient needs are fully addressed&nbsp;</li>

</ul>

<ul>

<li>Deliver hands-on personal care and supportive services to assist with safe transfers and ambulation&nbsp;</li>

</ul>

<ul>

<li>Thoroughly document care delivery daily in our Electronic Medical Record system&nbsp;&nbsp;</li>

</ul>

<li>Contribute to a culture of caring through individual accountability and teamwork&nbsp;</li>

<p><strong>Experience to Deliver on our Mission:</strong>&nbsp;</p>

<li>High school diploma or equivalent preferred&nbsp;</li>

</ul>

<ul>

<li>Certification required for State-specific certified aide certification&nbsp;</li>

</ul>

<ul>

<li>State licensure or a competency evaluation program, valid state driver’s license and auto liability insurance&nbsp;</li>

<li>One year experience as a Home Health or Hospice Aide or Nursing Assistant in a hospital, nursing home, or home health agency. Home health experience preferred&nbsp;</li>
