<p><em><strong>As a Registered Nurse, you will:</strong></em></p>

<ul>

<li>Evaluate and assess patient to determine their appropriateness for care</li>

<li>Utilize the available tools and resources to develop patient specific care plans with the appropriate level and frequency of care</li>

<li>Communicate with the care team, physician, payors, patients and families to address care needs and fulfill the patients care plan</li>

<li>Deliver high quality skilled care and supervise the care of other care team members</li>

<li>Thoroughly document care delivery daily in our EMR system</li>

<li>Contribute to a culture caring through individual accountability and teamwork</li>

<p><strong>Experience to Deliver on Our Mission:</strong></p>

<li>Current RN License, valid state drivers license, and reliable transportation</li>

<li>One year of experience as a Registered Nurse in an acute or clinical care setting preferred. Home health experience preferred</li>
