<p><strong>Key Responsibilities</strong></p>

<ul>

<li>Perform comprehensive initial and ongoing patient assessments to determine home health needs.</li>

<li>Develop, implement, and update individualized plans of care in collaboration with physicians and the care team.</li>

<li>Provide direct skilled nursing care, including medication administration, treatments, and preventive and rehabilitative interventions.</li>

<li>Identify changes in patient condition and communicate promptly with the physician to obtain updated orders.</li>

<li>Educate and counsel patients and families on care needs, disease management, and discharge planning.</li>

<li>Serve as case manager when assigned, coordinating care for an assigned caseload.</li>

<li>Complete accurate, timely documentation in the electronic medical record.</li>

<li>Participate in case collaboration, supervise home health aides (as assigned), and support care plan oversight.</li>

<li>Participate in on-call rotation as needed.</li>

<p><strong>Qualifications</strong></p>

<ul>

<li>Graduate of an accredited school of nursing.</li>

<li>Current RN license in state of practice and CPR certification.</li>

<li>One (1) year of home health experience preferred.</li>

<li>Strong clinical judgment, communication, and critical thinking skills.</li>

<li>Valid driver’s license, reliable transportation, and required insurance coverage.</li>
