Michigan City Coolspring Health Center 1225 E Coolspring Ave Michigan City, Indiana 46360 * Full Time, Part Time and PRN positions available* A Home Health Registered Nurse develops and oversees nursing care plans that individually address each patient’s needs

Practices nursing according to Franciscan Alliance and nursing divisions philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations

Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families Collaborates with patients and their families, as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience

Assesses, collects, and analyzes comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs and plan of care

Develops an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously review and revise the plan of care as patient condition warrants

Prioritizes and implements the identified plan, coordinates care delivery, employs interventions to improve patient outcomes while providing a safe, respectful, and ethical practice environment

Evaluates the individualized plan of care and progress towards attainment of outcomes in collaboration with patient and family utilizing available data and resources

Communicates timely changes in patient’s assessment/ongoing reassessment to providers that warrant consideration and/or a change in the plan of care and maintain and coordinate communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery

Maintains continuity among nursing teams by documenting the assessment, plan of care, interventions, and responses to care in the electronic medical record in a time

Promotes patients independence by establishing patient care goals; teaching patient, friends, and family to understand the condition, medications, and self-care skills; answering questions

Maintains a clean and safe work environment by handling and clearing instruments, equipment, and supplies in accordance with infection-control policies and protocols

Administers and stores medication according to controlled substance regulations Ensures operation of equipment by completing preventive maintenance requirements as per manufactures instruction or calling for repairs when required

Maintains nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies and placing orders for required inventory Supervises clinical care provided by LPNs and Home Health Aides

Serve in an on-call rotation and provides patient care outside of regular business hours.

QUALIFICATIONS
