The Home Health Registered Nurse administers skilled, quality nursing care to patients on an intermittent basis in their place of residence This position coordinates and directs the home health patient's care based on individual patient needs

This requires the use of advanced pediatric assessment, teaching and decision-making skills This is performed in accordance with physician orders and plan of care under the direction of the Administrator or designee

The Home Health RN provides comprehensive assessment, planning, implementation and evaluation for a caseload of home health patients in accordance with the plan of care

Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs by assessing physical, functional, psychosocial and cognitive status of the home health patient utilizing interview observations and physical exam techniques

Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames for applicable patients

Based on evaluations, determines the amount and type of nursing needed by each individual patient Establishes individualized, measurable goals in consultation with the patient, family and other health care providers

Work hours are primarily Monday-Friday 8-5pm There are times when we have evening visits that extend past normal operational hours RN is expected to cover the evening visit when needed

Weekend work is a shared responsibility among clinical nursing staff, with expectations distributed as evenly as possible On-call coverage supports patient needs outside of regular business hours, including evenings, weekends, and some holidays

On-call clinicians must be reachable and prepared to respond promptly and required patient visits are expected to be addressed within one hour Minimum Education and/or Training: Graduate from an accredited school of nursing

RN required BSN preferred.

Minimum Experience

Four (4) years of nursing experience required Pediatric experience preferred Licensure, Registration and/or Certification Required by Law: (LC: RN-TN) Must possess a current Tennessee State Board of Nursing license if primary residence is Tennessee or a Nurse Licensure non-Compact state.
