 Summary: Lincoln County Medical Center is seeking a compassionate and patient-focused Registered Nurse (RN) to join our outpatient clinical team
The Outpatient RN provides direct nursing care, patient education, care coordination, and clinical support to providers while promoting high-quality, patient-centered care in an ambulatory setting
This role serves as an integral member of the healthcare team, collaborating with physicians, advanced practice providers, medical assistants, and support staff to ensure efficient clinic operations and exceptional patient experiences

 Responsibilities: * Degree from accredited academic nursing program (If a program is under conditional status, approval may be granted by system CNO) * Minimum ADN preparation with expectation of BSN completion within 5 years of hire * Responsible for walk in and telephone triage. * Coordinates patients care through collaboration with the patient/family and healthcare team. * Provides patient care by assessing, planning, implementing, evaluating and documenting individual patient needs, actions taken and patient responses. * Responsible for case management based on PCP panels, in collaboration with network providers to include nurse visits, telephone management, etc. * Delegates or assigns aspects of patient care to healthcare team members commensurate with their validated competency to perform the task. * Documentation within scope of practice.
