 Responsibilities: As a Hospice RN , you will forge deep, meaningful relationships with patients and their families during life’s most tender transitions, delivering expert clinical skill along with deep compassion

Proactive Skilled Nursing: Execute physician orders, administer specialized interventions, and continuously monitor symptom management and patient response Collaborative Team Leadership: Lead interdisciplinary care coordination, guiding LPNs, CNAs, physicians, and community partners to ensure seamless support
Mentorship & Precepting: Shape the future of hospice nursing by precepting new hires and clinical students while serving as a trusted clinical resource Regulatory & Quality Assurance: Maintain pristine clinical documentation aligned with state, federal, and QAPI/PIP quality standards
Dynamic Caseload Management: Balance outpatient visits schedules in real time while consistently meeting department productivity and visit targets Reliable Patient Advocacy: Communicate schedule updates proactively, accommodating patient preferences and ensuring unwavering support for families
Flexible Care Continuity: Participate in rotating weekends, holidays, and shift coverage to guarantee uninterrupted, compassionate patient support.
